How To Register with the Practice

To register with the practice you must complete our “Registration Form” which is available at reception. We will then arrange an appointment with one of our doctors and by mutual agreement we will then add you to our system. If you are the holder of a medical card with another practice we will help you complete “Change of Doctor” form which is available at reception but please note that until your card has been transferred to us you must attend your previous doctor, this can take up to three weeks.

How to add new Dependent to a Medical Card

In order to add a new dependent to a medical card, you must send a copy of the new dependent’s birth certificate and the official P.P.S. Number notification together with a form that is available at Reception. Please note that under the Mother and Child Scheme your baby is only covered up to the age of six weeks after birth.

Lab Results Procedures

In cases where lab results necessitate further action or a review consultation with a G.P. the patient will be contacted directly by Kilnacourt Surgery. If you have any queries regarding your blood results please phone the practice nurse between 2pm and 3pm Monday to Friday. Please allow 7-10 days for all results to be returned to the surgery.

Medical Card Entitlements

Your medical card entitles you to a wide range of services from your family doctor.
There are however some services that are not covered by the medical card.

How to Apply for General Medical Cards

The quickest way to apply for a medical card is online, on www.medicalcard.ie.

MedicalCard.ie is a HSE website dedicated to providing you with online services to help you with your medical card.

On MedicalCard.ie you can:

1) Submit an online application, which is a faster route to your medical card than the manual paper forms. If your application is successful you will have your medical card within 15 days.

2) Check the current progress of your medical card application if this has been submitted to the central processing office.

3) Check the current status of your medical card.

4) Submit a query to the central processing office.

Make sure you have all the information you need to hand such as you PPS number and those of your dependents, a recent payslip, details of your income and outgoings on tax, PRSI, rent and mortgage etc.
